Each row is one command or behaviour: name recall, sit, down, stay, leave it, loose-lead walking, settle on a mat, the crate, the car, meeting other dogs. The first column is the behaviour, the second is the cue and the reward you use for it, so that everyone in the house says and does the same thing. Then come the weekly columns, each a small cell where you write the number of sessions and the minutes, like "5 / 20".

At the right-hand end sit five tick boxes under the heading Progress. They are not a grade; they are a quick way to mark where a behaviour is today, from "only at home with a treat in my hand" to "in the park with a dog walking past". Score it at the end of each block and you can see which behaviours are moving and which have been stuck at two for a month.

Choose four or six weeks before you download. Four gives wider weekly cells, which suits writing out sessions and minutes in a hurry; six covers a longer block of training on a single sheet. Either way you get twelve rows, a ruled box at the foot for what worked, what to change and what to ask the trainer or the vet, and a dog, trainer and start-date line across the top. Every cell is a PDF form field, so the log can be typed up or filled in with a pen by the back door. One landscape page in US Letter or A4.

How to use it

  1. 1

    Pick the number of weeks

Four for wider weekly cells, six to cover a longer block on one sheet. Then choose US Letter or A4.

  2. 2

    List the behaviours

One per row, with the exact cue word and the reward you use, so everyone in the household trains the same way.

  3. 3

    Print or fill it in

Print at 100% ("Actual size") so lines and boxes keep their exact spacing, or fill the form fields on screen in any PDF viewer.

  4. 4

    Log and score

Write sessions and minutes in each week's cell, then tick a progress box out of five at the end of the week.

Twelve rows, plus a ruled notes box at the foot. Most people find four or five behaviours at a time is plenty and use the spare rows as the earlier ones become reliable.
Whatever you decide, consistently. A common scale is: 1 at home with a lure, 2 at home without one, 3 in the garden, 4 outside with no distractions, 5 outside with distractions.
Yes. The dog's name, every behaviour, every weekly cell, the tick boxes and the notes box are PDF form fields that work in any normal PDF viewer.
No. It is a record sheet. It does not tell you how to teach anything — that is a question for a qualified trainer, and the notes box is there for what they tell you.
