Each row takes one medicine: its name and strength, the amount and form (half a tablet, 1 ml of the liquid, one spot-on), when it is due, and who in the house is giving it. Then seven tick boxes run across, one for each day of the week, so the sheet shows at a glance whether this morning's dose has gone in. Ten rows is enough for an animal on several medicines at once, which is usually an older one.
Everything is copied from the label, not from the internet. The sheet never suggests a medicine, an amount or an interval, because those depend on the animal's weight, species and condition, and some things that are safe for a dog are not safe for a cat. The note at the top says so plainly: ask your vet before you change, stop or double anything, and tell them about any reaction.
Two ruled boxes at the foot cover the questions a vet asks at the next appointment: reactions or side effects you saw, and doses that were missed and what you did about it. Pick whether the week starts on Monday or Sunday, fill in the pet's name, weight and the week, and print it for the kitchen wall. One landscape page, US Letter or A4.
