The first year is a blur of appointments, and the detail you need six months later is always the one nobody wrote down. The main table gives thirteen rows with columns for the date, the puppy's age, the weight that day, the reason for the visit, what was given or done, the next due date and the vet's initials. Logging the weight at every visit turns the same sheet into a growth record without any extra work.

Underneath sits a second, smaller table for worming, flea and tick treatments: date, product, the weight it was dosed for, and when it is next due. Keeping those apart from the vaccinations matters, because they run on their own intervals and because the dose is tied to a growing puppy's weight. Six rows cover the first months, and the top of the page holds the puppy's name, breed, date of birth, the day it came home, microchip, clinic, out-of-hours vet and insurance policy.

Both tables print blank on purpose. Vaccination and worming schedules differ by country, by breeder and by animal, so write down what your vet actually gives and what they say is due next. Every cell is a PDF form field, so the log can live on your computer and be printed whenever a kennel, a trainer or a new clinic asks for it. One page, US Letter or A4.

How to use it

  1. 1

    Download the log

Choose US Letter or A4 and download the one-page PDF.

  2. 2

Fill in the puppy and the clinic

Name, breed, date of birth, the day it came home, microchip number, your vet, an out-of-hours number and the insurance policy.

  3. 3

    Print or fill it in

Print at 100% ("Actual size") so lines and boxes keep their exact spacing, or fill the form fields on screen in any PDF viewer.

  4. 4

Add a line at every visit

Date, age, weight, what the visit was for, what was given, and the date the vet says the next one is due — that last column is the one that saves you a phone call.

Frequently asked questions

Yes. There is no watermark and no sign-up for your first 20 downloads each day; after that a free account keeps downloads going. You may print the file as often as you like for personal, school or business use.
No, and deliberately. Schedules differ by country, by breeder and by individual animal. Your vet decides what is given and when; this sheet records it.
Thirteen visit rows and six rows for worming, flea and tick treatments, which is enough for most first years. Print a second copy to carry on.
Two reasons: a puppy's weight is the record of how it is growing, and most worming and flea products are dosed by weight, so the figure on the day matters.
Yes. Every detail line and every cell in both tables is a PDF form field. Fill it in Adobe Reader, Preview, Chrome or Edge, save it, and print when someone needs a copy.
Yes. Nothing in the columns is puppy-specific apart from the title — the same visits, weights, treatments and next-due dates apply to any new animal.
