Eight boxes in two columns. The left column starts with the animals themselves — name, species and breed, age, sex, colour and markings, microchip and where they sleep — then gives ruled space for the daily routine with times, for food and water, and for medication: what, how much and when. The right column opens with who to call: you, a second contact, the vet clinic, an out-of-hours vet and the insurance policy number, each on its own line.

The three ruled boxes underneath are the ones that make a stay go smoothly. Habits, fears and warnings is where you write that the cat bolts when the doorbell goes, or that the dog is fine with everyone except the bin lorry. House rules and jobs covers the sofa, the bins, the plants and the alarm. Where things are kept saves your sitter opening every cupboard looking for the lead, the litter or the spare towels.

Each ruled box is a form field you can type into, so you can keep one filled-in copy on your computer, change the dates and print it for every sitter, or print it blank and write it out by hand before you leave. Nothing is pre-printed except the headings, because no two households run the same way and a list of somebody else's rules is no use to yours. It is one page by design: a sheet that fits on a fridge door gets read, and a four-page folder does not. US Letter or A4.

How to use it

  1. 1

    Download the sheet

Choose US Letter or A4 and download the one-page PDF.

  2. 2

    Fill in the contacts first

Your number, a second contact, the vet clinic, an out-of-hours vet and the insurance policy — these are the lines that matter most if something goes wrong.

  3. 3

    Write the routine as times

"Out at 7, fed at 7.30, walk at 5" is easier to follow than "twice a day". Add medication amounts exactly as the label gives them.

  4. 4

    Print or fill it in

Print at 100% ("Actual size") so lines and boxes keep their exact spacing, or fill the form fields on screen in any PDF viewer.

Frequently asked questions

Yes. There is no watermark and no sign-up for your first 20 downloads each day; after that a free account keeps downloads going. You may print the file as often as you like for personal, school or business use.
Yes. The animal details and contact lines are single-line fields and each of the six ruled sections is a multi-line field, so you can fill the whole sheet on screen, save it and send it to your sitter as well as printing it.
The details box is sized for one, but the ruled sections have room for a household — many people write "both" or use the animals' names in the routine and food sections. For very different routines, print one sheet each.
Anything that would surprise someone new: a door that must stay shut, a dog that guards food, a cat that hides for the first day, a fear of thunder, a lead that must go on before the front door opens.
It gives you a section to write them in. It does not suggest any medicine or dose — copy those exactly from the label your vet gave you.
Yes. It is one page, so you can print it back to back with the pet feeding schedule and hand your sitter a single sheet of paper.
