Twelve rows, one per medicine, run across seven day columns. Each day is split into dose boxes lettered M, D and E for morning, midday and evening, or just M and E if you choose the two-dose version, giving up to twenty-one tick boxes per medicine per week. Two further columns hold the refill date and the prescriber, which are the two things people most often have to phone a surgery to ask about.

The medicine and dose columns are deliberately wide, because the instruction on the sheet is to copy the name, the strength and the dose exactly as they appear on the label rather than from memory. Nothing is pre-printed: no drug names, no doses, no schedule. It is a reminder and a record, and the header says in one line that any change belongs to your prescriber or pharmacist, not to a chart.

All 302 boxes and fields are fillable, so a carer can type the week up and print a clean copy for the kitchen, or for the person who takes over at the weekend. The landscape page prints on US Letter or A4 and is worth taking to appointments, because a current list in one place is usually more accurate than a memory under pressure.

How to use it

  1. 1

Choose two or three doses a day

Two gives morning and evening boxes; three adds a midday column for each day.

  2. 2

    Copy from the label

Type each medicine, its strength and its dose exactly as printed on the box or the pharmacy label.

  3. 3

    Print or fill it in

Print at 100% ("Actual size") so lines and boxes keep their exact spacing, or fill the form fields on screen in any PDF viewer.

  4. 4

    Tick each dose

Mark the box as the dose is taken, and keep the sheet with the medicines so the record stays honest.
