Appointments are short and symptoms are easy to under-report once you are in the room. This sheet is built for the week or month before one: eighteen rows with seven columns each, covering the date, the time of day, the symptom itself, how severe it was, how long it lasted, a possible trigger and what, if anything, helped. Written at the time, those columns make a pattern that memory usually flattens.

The header has fields for your name, what you are tracking, the appointment date and the clinic, so the sheet identifies itself if you hand it over or it ends up in a file. At the foot, a panel headed questions to ask at my appointment gives you four checkbox lines, because the question you wanted to ask is the one that disappears while you are getting your coat.

Severity is written out of ten in your own judgement; it is your own scale rather than a clinical score, and nothing on the sheet interprets it or sorts your entries into categories. All 138 fields are form fields, so the diary can live on a phone and be printed the night before the appointment, or be printed blank and carried in a bag with a pen. It is one page in US Letter or A4, and a second copy picks up where the first one runs out.

How to use it

  1. 1

    Say what you are tracking

Fill in your name, the symptom or condition, the appointment date and the clinic at the top.

  2. 2

    Print or fill it in

Print at 100% ("Actual size") so lines and boxes keep their exact spacing, or fill the form fields on screen in any PDF viewer.

  3. 3

    Write entries as they happen

Record the time, how bad it was out of ten, how long it lasted, a possible trigger and anything that helped.

  4. 4

    List your questions

Use the four lines at the foot for the questions you want answered, and tick them off as they are asked.

Eighteen rows per sheet, plus four questions at the foot. Print a second copy if you are tracking something that happens several times a day.
Out of ten, in your own judgement, and the same way each time so the entries can be compared. It is not a medical score.
Yes, every cell and both panels are form fields, so you can fill it on a phone or laptop and email the file to yourself before the appointment.
Bring the sheet. Clinicians generally prefer to see the entries and the dates, and the diary is designed to be read across a desk.
No. It is a blank sheet for recording what you measure or do, and it prints no targets, no normal ranges and no doses. Anything you write on it is for you and your doctor, nurse or pharmacist to interpret together.
