Thirty-one numbered rows run down the page. In each row five faces are printed, from very low to great, and you circle one. Because the faces are drawn rather than written, the sheet takes about four seconds a day to fill in, which is the difference between a tracker that survives a month and one that stops on the ninth.

Next to the faces is a narrow column for the hours you slept and a wide one for what shaped the day: the deadline, the argument, the long walk, the person you saw, the medication change. Sleep and mood move together often enough that having the two side by side is usually the first pattern anyone notices, and a trigger written on the day it happened is worth more than a week of trying to remember which Tuesday was the bad one.

A legend at the top shows what each face means, and the month and your name are fillable. The faces are printed, not clickable, so this is a sheet to print and mark with a pen, though the sleep and notes columns are form fields if you prefer to type. It is one page in US Letter or A4. A diary like this records a pattern rather than explaining it: if low days are stacking up, take the page to your doctor — and if you are having thoughts of harming yourself, contact your doctor or a local crisis line now rather than waiting for the month to finish.

How to use it

  1. 1

    Download and print

Pick US Letter or A4 — one page covers a whole month.

  2. 2

    Fill the month in

Type the month and your name before printing, or write them by hand.

  3. 3

    Print or fill it in

Print at 100% ("Actual size") so lines and boxes keep their exact spacing, or fill the form fields on screen in any PDF viewer.

  4. 4

    Circle a face each evening

Add the hours you slept and one line on what shaped the day.

Frequently asked questions

Yes. There is no watermark and no sign-up for your first 20 downloads each day; after that a free account keeps downloads going. You may print the file as often as you like for personal, school or business use.
No. The faces are printed artwork so they stay the same on screen and on paper; the sleep and notes columns are fillable fields. On a tablet you can circle a face with a stylus in any annotation app.
So one sheet works for every month. In a shorter month leave the last rows empty.
One concrete thing, not a summary — "slept badly, deadline moved" is more useful in three weeks than "bad day".
Yes. Open the PDF in GoodNotes, Notability or Xodo and write on it with a stylus, or fill the form fields and keep the file on the device instead of printing it.
No. It is a blank sheet for recording what you measure or do, and it prints no targets, no normal ranges and no doses. What you write on it is for you and your doctor, nurse, therapist or pharmacist to read together.
