Home exercises get done when they are written down somewhere you will see them. The ruled panel at the top of this sheet is for copying the plan your physiotherapist handed you — the exercises, the sets, the band, the things they told you to stop doing — so it is in your handwriting and on the same page as the log.

Under it, a table of eighteen or twenty-four rows takes the day's work: date, exercise, sets, reps, hold or band, a pain score before, a pain score after, and notes for what the therapist said or what you changed. The two pain columns are the useful part of the sheet. A number before and a number after, written at the time, turns a vague feeling into something you can show at your next appointment.

The pain scores are your own nought-to-ten judgement, not a medical score, and the sheet prescribes nothing: the exercises, the numbers and the decision about when to progress all come from your therapist. Every cell is a form field, so you can fill it on a phone between sets or print it blank and keep it on the floor next to the mat. One page in US Letter or A4, with the row count chosen before you download.

How to use it

  1. 1

    Copy the plan

Write your therapist's exercises and instructions into the panel at the top of the page.

  2. 2

    Choose the row count

Eighteen rows for roomy handwriting, twenty-four if you log several exercises a day.

  3. 3

    Print or fill it in

Print at 100% ("Actual size") so lines and boxes keep their exact spacing, or fill the form fields on screen in any PDF viewer.

  4. 4

    Score the pain

Write a nought-to-ten number before and after each exercise, then take the sheet to your next appointment.

Yes. There is no watermark and no sign-up for your first 20 downloads each day; after that a free account keeps downloads going. You may print the file as often as you like for personal, school or business use.
Eighteen or twenty-four, whichever you choose before downloading. Most people use one row per exercise per session.
Whatever your own nought-to-ten scale means to you, used consistently. It is not a clinical score, and a therapist reads it alongside what you tell them.
Yes. The four header fields, the plan panel and every cell in the table are fillable form fields.
Yes. Choose two-sided printing in the print dialog and one sheet gives you two pages of the same layout. Nothing is printed upside down or mirrored, so both sides read the same way.
No. It is a blank sheet for recording what you measure or do, and it prints no targets, no normal ranges and no doses. What you write on it is for you and your doctor, nurse, therapist or pharmacist to read together.
