Six sections and 46 printed items, arranged in the order a new owner meets them. Equipment covers the crate, bedding, collar, harness, long line and bowls. Food and feeding covers keeping the breeder's food for the first week, an airtight bin, training treats and chews. Vet and health covers the first appointment, the vaccination record, worming, microchip registration and insurance. Every section ends with a blank line, because every dog arrives with something the list did not predict.

The last two sections are the ones people forget. Puppy-proofing walks the house: cables, cleaning products, houseplants, a bin with a lid, a stair gate, the garden fence. "The first week, day by day" turns the first seven days into tasks — a quiet house on day one, the crate shown and left open, out to the toilet every hour and after every meal, sleeping within earshot, and writing down meals, toilet trips and sleep so you can see the pattern forming.

Two write-in boxes sit at the foot of the page: one for vet, microchip and insurance details, one for notes. The puppy's name, breed, homecoming day, both boxes, the blank lines and every checkbox are PDF form fields, so you can fill it in on screen and print a copy for the fridge, or print it blank and tick with a pen. One page, US Letter or A4.

Most of it does. The equipment, food, vet, puppy-proofing and first-week sections apply to any new dog; the training section is written for a puppy, so use the blank lines to swap in what an older dog needs.
Yes. The puppy's name, breed, homecoming day, the blank lines, the two write-in boxes and all the checkboxes are form fields that work in any normal PDF viewer.
No. It reminds you to keep whatever the breeder or shelter was feeding for the first week and to talk to your vet about changing it. Amounts and brands are a question for them, not for a printable.
