This is the page that goes on the fridge or inside a kitchen cupboard, where anyone in the house can find it without unlocking a phone. Nine sections give you 34 contact lines: emergency services, doctors and health, family and emergency contacts, neighbors and friends, school, childcare and work, home and insurance, utilities, and pets. Each line has room for a name and a phone number.
Some lines come labeled — police, fire and ambulance, the non-emergency police line, poison control, the nearest hospital, the pharmacy, home and car insurance, the gas, electricity and water suppliers, the vet — and the rest are blank for the people particular to your household. The header takes your home address and phone numbers, which matters when the person making the call is a visitor who does not know the street name.
A large box at the bottom holds instructions for a sitter or caregiver: allergies, medicines, bedtimes, where the first aid kit is. Every line and box is a form field, so you can type the list once, print copies for the fridge, a school bag and a grandparent, then update the file next year instead of rewriting it. One page, US Letter or A4.
