Calligraphy guide sheets are measured in nib widths, not millimetres, because letter height has to follow the pen you are holding. Each line group here gives an x-height of five nib widths with three nib widths of ascender above and three of descender below, which is the standard proportion for italic and foundational hands. Choose your nib and the whole ruling rescales around it.
A 2 mm nib gives ten line groups on a US Letter page, a 3.5 mm nib gives five and a 5 mm nib gives four. The four rulings in each group are weighted so you can read them at a glance: a dark baseline, a mid-gray waist line at the top of the x-height, and light ascender and descender lines. Slant lines cross every group at 55 degrees, the classic italic angle, spaced one x-height apart so you can keep your downstrokes parallel.
This is a practice sheet, not a fill-in form — print it and write on it with a broad-edge pen, a brush pen or a pencil. Many calligraphers slide a printed guide under a sheet of layout paper instead and trace the lines through, which keeps the finished piece clean. Print at 100%, or the nib-width maths stops working and your letters will not match the rulings.
