The page is laid out exactly as the method is taught: urgent and not urgent across the top, important and not important down the side, with the four quadrants numbered in the order you should work them. Each quadrant is labelled twice, once with the action — do first, schedule, delegate, drop — and once with the combination it stands for, so nobody has to remember which box is which.
Every quadrant holds eight lines, each with a checkbox, which is 32 tasks in total. That is roughly the right size: enough to empty a messy to-do list onto the page, small enough that you have to decide rather than copy everything across. The lines are form fields, so you can sort the list on screen and print the result, or print it blank and sort with a pen in a meeting.
It is one landscape page with a light lavender heading on each quadrant and gray rules elsewhere, so it prints clearly in black and white and photocopies for a team. Use it weekly on your own backlog, or draw it up with a group when everything claims to be a priority. The date field at the top right means a stack of filled sheets stays in order when you look back over a month of decisions.
