Most one-to-ones fail in the same way: things get agreed and nobody writes them down, so the next meeting starts from scratch. This sheet opens with a carried-over table - three rows for the action from last time, its owner, when it was agreed, where it stands and a done box - and closes with the actions agreed today. Copy anything unticked onto the next sheet and an action cannot quietly disappear.
The guided version prints five prompts with a writing box under each: progress and anything to celebrate since last time, what is in the way, workload and priorities for the next few weeks, feedback in both directions, and development, training and career. They are deliberately in that order, so the conversation reaches development instead of running out of time at blockers. The open version drops the prompts for two large boxes instead, one for the report's agenda and one for the manager's.
The header takes both names, the date of this meeting, the date of the next one and a meeting number, which is how a run of sheets becomes a record rather than a pile. The closing table has four rows for the action, the owner, the date it is due and a tick box. Everything is a form field, so a manager can type notes during the meeting or print a stack of blank sheets for a notebook. One page, US Letter or A4.
