The sheet is laid out the way the walk-round is done. The left column covers the outside: lights front and rear, brake and reversing lights, plates, mirrors, glass, wipers and washers, bodywork and steps, tyre tread and pressures, wheel nuts, leaks under the vehicle, the load area and doors, markings, mud flaps and the fuel cap or charge point. The right column covers the cab and under the bonnet. Each check has an OK box, a defect box and a short notes field.
Choose the van version and the cab list covers the horn, belts, steering, brakes, dash warning lights, gauges, heater and demister, fuel or charge, oil, coolant, screen wash, battery, spare wheel and the documents, first aid kit, warning triangle and high-visibility kit. Choose the truck version and it swaps in air pressure build-up, air lines and electrics, the fifth wheel or tow hitch, load security and straps, the tachograph and limiter, and spare bulbs and chocks.
Below the two columns, a large box takes the defect report itself - what it is, where it is and how bad - because a tick in a defect column means nothing on its own. Tick boxes record whether the vehicle is fit to drive or must not be used, a line records who the defect was reported to, and the page ends with the driver's signature, a line for whoever checks or repairs it and a date. The header takes the registration, make and model, fleet number, driver, date, time and odometer.
