Petty cash goes missing when nobody writes down where it went. A voucher fixes that: one slip per payment, numbered, signed, and dropped in the tin so the cash plus the vouchers always add back up to the float. This sheet prints three vouchers to a page, or two if you want more room to write, with a dashed cut line between them.

Each voucher has a number and date in the header, then the amount and the account or cost code it should be charged to, who the cash was paid to, and the date the money was actually spent. Tick boxes record whether a receipt is attached and whether any change came back. A bordered box takes the purpose — what the cash was spent on — with enough space to write a sentence rather than one cramped word.

At the foot of every voucher are two lines: approved by, and received by with a signature. That pairing is the whole control — one person releases the cash, another signs for it. All of it is fillable, so you can type a voucher and print it, or print a sheet of blanks and keep them beside the tin. Amounts are labelled without a currency symbol, and the page prints at full size on US Letter or A4.

How to use it

  1. 1

Choose two or three per page

Three to a page is the usual size for a cash tin; two gives a larger purpose box.

  2. 2

    Number and date each voucher

Keep the numbers running in order so the vouchers in the tin can be checked against the float.

  3. 3

    Record the payment

Amount, account or cost code, who was paid, what it was for, and whether a receipt is attached.

  4. 4

    Get it approved and signed

The approver signs the release, the person taking the cash signs to say they received it. Cut along the dashed line.

No. Figures are typed by hand — the form does not add anything up, so it behaves the same in every PDF viewer.
The money columns carry no currency symbol, so the same sheet works wherever you trade. Write the currency once at the top if it is not obvious.
Yes. Every line, tick box and the purpose box is a form field, so you can fill one on screen and print or email it.
Keep it with any receipt in the cash tin until the float is reconciled, then file the batch with the period it belongs to.
