Ten columns across a landscape page: the date, the visitor's name, their company, a vehicle registration if they drove in, who they are visiting, the reason for the visit, the time in, the time out, the badge number they were handed and a tick for signed out. The header records the site or building, the month and a sheet number, so a run of sheets stays in order in the folder at the desk.

The sheet is designed around the moment it actually matters. If every visitor is ticked out as they leave and the badge comes back, the lines without a tick are the people still inside - which is what you carry to the assembly point in an evacuation and read out. That is printed under the table as a reminder, along with the advice to keep completed sheets at reception rather than filing them somewhere clever.

Choose twenty rows for a roomier line to write on or twenty-six for a busy reception. Every cell is a form field as well, so a receptionist with a tablet can type entries straight in, although most sites print the sheet, clip it to a board and let visitors write. It works for an office, a site hut, a school reception, a warehouse gate or a village hall. One page, US Letter or A4, landscape.

How to use it

  1. 1

    Choose 20 or 26 rows

Twenty rows give more room per line; twenty-six suit a busy front desk.

  2. 2

    Download the PDF

US Letter or A4 in landscape, free and without a watermark.

  3. 3

    Print or fill it in

Print at 100% ("Actual size") so lines and boxes keep their exact spacing, or fill the form fields on screen in any PDF viewer.

  4. 4

    Sign them in and out

Hand over a badge and write its number, then tick the out column and take the badge back when the visitor leaves.

Frequently asked questions

Yes. There is no watermark and no sign-up for your first 20 downloads each day; after that a free account keeps downloads going. You may print the file as often as you like for personal, school or business use.
Twenty or twenty-six, depending on the version you choose. The sheet number field at the top keeps a day of several pages in order.
Every cell is a form field, so it can be filled in on a tablet, but most receptions print it and let people write. Printing also avoids one visitor reading the previous entries on a screen.
Landscape, because ten columns need the width. It still prints on a standard US Letter or A4 sheet.
No. The sheet collects only the basics and names no country's privacy law; how long you keep completed sheets, and who can see them, is your own policy to set.
That is exactly what the signed-out column is for: the lines without a tick are the visitors still in the building. Keep the current sheet somewhere you can grab it on the way out.
