A delivery note travels with the goods and settles the only question that matters at the door: did everything arrive, and was it in one piece. The header carries a delivery note number, the delivery date and the order or reference number it belongs to. Underneath sit two blocks — who sent the goods and who is receiving them — followed by a strip for the driver's name, the vehicle and its registration, and the time the van left and arrived.
The body is a fourteen-row table with columns for item description, code or SKU, quantity, number of packages, and condition on arrival. That last column is the point of the sheet: a driver and a receiver can mark a crushed box or a short count while both are standing there, instead of arguing about it a week later. A notes box below catches damage, shortages and items still to follow, next to tick boxes for goods received in full or in part and a line for the package count.
The sheet ends with a signature and date for the driver, then a signature, printed name and date for whoever accepts the delivery. Every line and cell is a real form field, so you can type the note from the order, print two copies and have both signed, or print a stack of blanks for the van. There are no prices anywhere — a delivery note records goods, not money. It prints at full size on US Letter or A4.
